Who we are
TruckApply operates truckapply.com, a service that matches commercial drivers with trucking companies that are hiring. We are the entity responsible for the data described here. Reach us at contact@truckapply.com or (708) 852-3088.
What consumer health data we collect
Four of the questions we ask a driver may be “consumer health data” under your state's law. We collect them directly from you and from nowhere else:
- Whether you are currently in a SAP program. A Substance Abuse Professional program is the federal return-to-duty process that follows a drug or alcohol testing violation. Answering yes reveals information about substance use.
- Your DOT medical certification status — valid, expired, or none. This is a statement about your physical fitness to drive.
- How many accidents you've had in the last three years.
- Whether you have a DUI or other major violation, and anything you told us about it, including in the free-text box or during our phone interview. That interview is not recorded.
We collect nothing else that could be health data. We do not collect diagnoses, prescriptions, treatment records, test results, biometric data, precise location, or anything about reproductive or gender-affirming care. We do not derive or infer health information from anything else you tell us.
Why we collect it
One purpose only: to avoid matching you with trucking companies that will not consider your situation, and to let the companies that do consider it see the facts up front. Carriers differ widely on whether they hire SAP candidates or drivers with a major violation. Knowing the answer means you hear from companies that might actually hire you instead of repeating the conversation ten times.
We do not use it for advertising, we do not use it to build a profile of you, and we do not use it to train any model.
Who we share it with
- Trucking companies we match you with — but only if you gave us separate permission for these specific answers, and only companies whose stated hiring policy actually considers them.
- Service providers who run our systems on our behalf: website and database hosting and telephone service. They are contractually limited to doing what we hired them for and may not use or sell your information.
- Nobody else. We do not share it with advertisers, data brokers, list vendors, insurers, or affiliates. We have no affiliates.
Selling it requires your signature
How long we keep it
Three years from the day you submitted the form, then deleted. If you ask us to delete it sooner, we do, including from our backups on their normal cycle. Any authorization you signed is kept for six years as required, separately from your profile, and is used only to show what you authorized.
Your rights
- Confirm and see it. Ask us whether we hold consumer health data about you, see it, and get the list of every third party and affiliate we shared it with — including how to contact each one.
- Withdraw your consent. Take back your permission to collect it, to share it, or both, at any time.
- Delete it. Ask us to erase it. We will, and we will tell every party we shared it with to do the same.
- No retaliation. Nothing about your matching changes because you did any of this.
To do any of these, email contact@truckapply.com with “Health Data Request” in the subject line, or call (708) 852-3088. We confirm within 10 business days and finish within 45 days. It is free. If we deny a request we will tell you why and how to appeal, and if we deny the appeal we will tell you how to complain to your state Attorney General.
Geofencing
We do not use geofencing of any kind. We do not track your location, and we never place a virtual boundary around a health care facility or anywhere else.
Changes to this notice
If we ever collect, share, or sell a category of consumer health data that is not described above, we will update this notice and get your consent before doing it — not after. We keep every prior version and will send you the one that was in effect on any date you ask about.
